Spousal Waiver Forms

Province
Alberta

Required form - Form 6 - Pension Partner Waiver on Transfer to a LIF, DC RIA or Annuity External site. Must be signed by spouse before money can be transferred to an annuity. Submit the consent form with the application.
British Columbia

Required form - Form 2 - Spouse's Waiver of Entitlements Under a Pension Plan, an RRSP, a Life Annuity or a LIF Contract External site. Must be signed by spouse before money can be transferred to an annuity. Submit the consent form with the application.
Manitoba

Required form - Form 1 - Pension Waiver (Joint and Survivor 66 per cent) External site. Must be signed by spouse before money can be transferred to an annuity. Submit the consent form with the application.
Ontario

If the spouse requests to waive his/her rights to the death benefit payable if the Annuitant dies prior to the conversion of the LIF to an annuity, obtain Life Income Fund (Ontario) - Spousal Waiver of Survivor's Benefits under a LIF (XPS BIL 3301-06-99) and submit with the application.

Proof of age is required for all Annuitants.
